MAPLE HEIGHTS TEACHERS’ ASSOCIATION
General Information to the Applicant:
The Maple Heights Teacher’s Association will award one laptop to a high school or post secondary student who plans to attend an accredited institution in the 2016-17 school year. The parent of the student must be a current MHTA member.
In order to qualify, the student must have maintained a 2.5 average in high school or at the post secondary institution that he/she is currently attending.
Recipients will not be determined on the basis of the type of accredited institution which they plan to or currently attend, i.e., trade school, college, university, school of nursing or on the amount of tuition charged by the institution, or financial awards received. Before the final award is made, the recipient must show acceptance to an accredited institution.
To apply for this laptop, complete the student application form and submit a one page essay on your career goals, personal needs for the laptop, and why you would be a good candidate for this laptop.
Please complete the information requested on the two personal references forms and submit them to two persons from who you wish references. References may not be relatives. Be sure to supply them with a stamped envelope so they can mail the completed forms to the scholarship committee chairperson by Friday, April 8, 2016.
Return the completed application, the essay and a copy of your high school transcript or most recent college report card to the scholar ship committee chairperson no later than Friday, April 8, 2016. It is responsibility of the applicant to make sure the committee receives all the forms prior to the deadline.
